Ed Wagner's Planned Care or Chronic Care Model is instrumental to Whatcom County's approach to Pursuing Perfection. To the right I rearrange the parts to put the patient in a more central role. All changes in clinical outcomes must move through the patient, through patient behaviors. That is the gist of my rearrangement.
